Grasmere Place is a Specialized Mental Health Rehabilitation Facility (SMHRF) located on the north side of Chicago. The purpose of this program is to provide support services to individuals who might otherwise have been referred into long-term care. Grasmere Place is committed to providing a comprehensive approach to psychiatric and mental health rehabilitation and offers skills groups seven days a week.

The Grasmere Place team manages each resident’s treatment and continuity of care according to the individual’s needs and our professional staff meets regularly to review everyone’s goals to assist with community living.

Services include but are not limited to Stabilization and Socialization, DBT (Dialectic Behavior Therapy), Anger Management, Community Re-entry, Friendship and Intimacy, Medication Management, Substance Abuse groups, Pregnancy Program, and many others to fit the need of the individual.

We are approved for Medicaid/Managed Care and Private Pay. Grasmere Place’s admission criteria are for residents with a mental illness diagnosis, age 18 to 62 years of age, PASRR screen, the ability to ambulate without assistance, and perform ADLs daily independently.
